I can assist organizations in developing, implementing, and maintaining Safety Management Systems in accordance with the Canadian Aviation Regulations, ISO 45001, and relevant Occupational Health and Safety legislation and regulations. 

I also perform incident/accident investigations to identify root and contributing causes, thereby enabling organizations to implement effective control measures to prevent recurrence.

My process is to start with a needs assessment. It is important to identify what the organization already does to manage safety in the workplace. Rarely would an organization not have some if not most required components in place. The purpose is to identify what is missing and what needs to be improved while keeping the changes to how things are done to a minimum. This is the best approach to ensuring any required change is easily integrated into the workplace.

An investigation must clearly define what happened, what was supposed to happen and identify why there was a difference.

My investigations look at the 

  • the event
  • the company's systems, processes and procedures
  • regulatory and industry standards
  • Industry best practice

Though employees remain responsible for their actions, they do not necessarily have control over their workspace or the task. Typically, investigations reveal employees did not intend the outcome and thought they were doing everything right.  A thorough investigation will identify the conditions that enabled the event to occur and provide recommendations for preventing recurrences.
